How Do I Register With The Practice?

If you live within our practice area you can register with us and our Reception staff will be happy to guide you through the procedure. Eligibility can be quickly and easily confirmed from your address so we ask that you provide proof by way of a recent utility bill. We will also require one form of photographic ID such as a passport.

You will need to complete one of our registration forms (GMS1) and a health questionnaire (both documents can be downloaded from the list below) which will provide useful information whilst we wait for your medical records to arrive from your previous Doctor.

If you are on any repeat medication please bring along a prescription slip from the Doctors you are currently registered at, when possible please try to ensure that you have a full months prescription from your current Doctors Surgery, this helps while we are awaiting your notes to come through.

Medical treatment is available from the date of registration. Please contact our reception for further information.

 

Out of Area Registration

From April 2018 we are unable to keep patient's who move outside our practice boundary, this is due to the influx of new houses that are being built within our boundry. This isnt a decision we have taken lightly but want to ensure we are offering our patients the best available care.

Please note: Patients going through treatment plans will be give a grace period of between 3 to 6 months to ensure that patient care and continuity is fulfuilled.  

Temporary Registration

If you are ill whilst away from home or if you are not registered with a Doctor but do need to see one you can receive emergency treatment from the local GP practice for a period of 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. Please note that you cannot register as a temporary patient at a practice in the Town or area where you are already registered.

Alcohol Questionnaire

Our Surgery are currently making use of a personal questionnaire that all patients can complete. This helps you, the patient understand whether you are drinking alcohol to a level that could be harmful to your body.
